When are applications due? How should they be completed?

Applications must be received or postmarked on or by October 15 each year. All applications should be typed and signed by the applicant. Programs must be specifically described with the names, artist fees, and qualifications of the individual (s) leading the activity and please note if there is any collaboration within our community.
