Each participant must provide a record of their most recent physical exam and immunizations dated within the past 24 months. Please bring all forms the first day of each new camp/clinic week for all programs. Please hand in forms to the camp director the first day of the program. If you do not have these forms your child will not be allowed to stay per board of health regulations.
This updated form allows the vendor to complete the booking sheet on the computer. Once all of the boxes have been fill in, please save and email to comeaua@sudbury.ma.us or print and mail a copy to us.

This policy applies to all Town of Sudbury and school fields K-8 (after school hours), athletic fields and Fairbank Community Center located in the Town of Sudbury, Massachusetts when scheduled by the Park & Recreation Department. All guidelines included in this policy must be met or the Park and Recreation Department has the right to revoke use of requested facility by applicants.

In 2003 Gale Associates was engaged by the Friends of Sudbury Park & Recreation to assist with an updated Athletic Needs Assessment and Master Plan. In 2012 Gale Associates was hired again to assist with updating of the 2003 plan. The update was needed due to changes in field inventory and condition and changes in community's needs.
