Summer Dear Parents:

The summer is quickly approaching. We are very excited for this summer and cannot wait for your children to participate in our summer programs. Sudbury Park and Recreation has been providing quality summer programming for several years and we are taking part in a very important initiative this summer that we wanted to make you aware of.

This letter is to inform you that all of our summer camps and programs being offered will be officially licensed through the Massachusetts Board of Health Department. While we have always done our best to make sure that each organization we work with hire the best staff and ensure the highest quality program possible, this year we are formalizing our compliance with the Board of Health to bring you the most fun and safest programs possible.

The Commonwealth of Massachusetts requires that you provide the following information for your child to attend ANY summer camp or program:

An up-to-date Physical Examination (within 2 years of the camp your child attends)
• Current Immunization Records
o MMR: 1st Dose at 1 year or older
o Measles: 2nd Dose at grades K-12
o Polio: 3 Doses OPV or IPV OR 4 Doses mixed OPV/IPV
o Diphtheria (DtaP/DTP/DT): 4 Doses
o Hep B: 3 Doses if born on or after 1/1/1992

You will need to submit the information listed above to the site coordinator for EACH program your child is registered for. For example, if your child is registered for a Skyhawks program and a week of Play Soccer, you must submit this information to each site coordinator. Please also inform the site coordinator of any allergies and medical or behavioral conditions that you child may have.

You will need to submit this information to the site coordinator on the first morning of each program (typically Monday mornings). Due to licensing requirements, your child will not be allowed to participate in camp if these documents are not on file!!!
