Below are Adult and Children&#8217;s Insurance Forms. Please click on the link below to download the appropriate form. One form is required per person: Please read all information carefully prior to clinic. Oct. 7th 1:00 pm-4:00 pm<\/li>\n<li>Clinic 2. Town Wide Sat. Oct. 17th&nbsp; 10:00 am-4:00 pm<\/li>\n<li>Clinic 3. Town Wide Wed. Oct 21st 1:00 pm &#8211; 4:00 pm<\/li>\n<\/ul>\n<p>It is projected that Flu Shots will be in high demand this Flu season.
