All Veterans that have a Massachusetts Disability Annuity must submit a W9 form, Residential Certification(to be done by the Sudbury Town Clerks Office) and the Annuity Certification which must be filled out by you. This must be sent in by October 29th in order to keep receiving your Annuity. Failure to do so will discontinue, or interrupt Annuity payouts. Contact our office for assistance. Read full article→
