The Sudbury Police Department has been receiving between 5 to 10 complaints daily recently of identity theft via a fraudulent unemployment claim.  To try to streamline the process we have created a form for residents to quickly and easily file their report with us.  There is also a resource brochure to help with other questions you may have.  Please fill out the attached form and email it to us at Police@sudbury.ma.us.  If you are unable to email it you are welcome to mail it to us at 75 Hudson Rd., Sudbury, MA  01776.  The report will be assigned to an officer and you will be contacted if further information is needed.
