1095-C Tax Form – New Option for Employees

In December 2024, President Biden signed legislation that allows employers to furnish employees Form 1095-C form upon request, rather than distributing to all employees.  Unlike the W-2 form, this form is not required by the IRS to file your yearly income taxes.  The Town, as required by the IRS, does report this information.  If you would like a copy of your 1095 form for your records, please contact the Payroll office at rowek@sudbury.ma.us and we will be happy to provide you with a copy.

Please note: if you are a Massachusetts resident, this form does not replace the MA 1099-HC.  The MA 1099-HC is still required for your state tax return and is provided by your health insurance carrier.
