It is that time of year already! The Holiday Gift Program applications are out and are due back in the Board of Health Social Work Office by 4pm on Friday, October 4, 2019. Please click here for a copy of the application.
The application is available to any individual or family who identifies as being in need this holiday season.
Please be sure to read all directions very carefully. Incomplete applications may be deemed ineligible. Applications MUST be received by the Board of Health Social Worker no later that 4pm on Friday, October 4th. Late applications cannot be accepted.
