This is a reminder that Avalon will be providing a number of affordable housing units in the new Meadow Walk development near Whole Foods on route 20 in Sudbury. For information on income limits, cost of rent and the application process, please click here.

This is a lottery system, and ALL APPLICATIONS ARE DUE BY FEBRUARY 15, 2018 AT 5PM. No late applications will be accepted. Please do not delay in submitting your application if you are interested in this program.

The Town Social Worker provides resources, referrals, and public health education to town residents of all ages.

When contacting the social worker, please note that the Secretary of State has declared that email is a public record and thus not confidential.

Some Social Work Services Provided:

Referrals to services related to: mental health, medical, parenting, housing, finances, basic needs, etc.
Providing information on area resources and services for the elderly, individuals with disabilities, children and families, low-income individuals, and those with a variety of other specialized needs
Connecting residents with community supports to help complete applications for services such as: food stamps, social security disability, fuel assistance, DMH (Department of Mental Health), Mass Health, etc.
Accepting and reviewing applications for the HOPE Sudbury fund
Connecting residents with volunteer and donation opportunities
Providing community education
Collaborating with community groups to address community needs and concerns, and to strengthen community connections
